Brief Description of Job Responsibilities:

Providing exceptional care for our patients as well as recognizing the value of your co-workers is the expectation of all members of the Prevea Clinic organization. All members are expected to focus on the patient`s needs; relate to all in a friendly, accepting manner; communicate in a positive and professional way to patients and co workers; use time effectively and efficiently; and demonstrate an overall high level of performance.

Greet, instruct, direct and schedule patients and visitors. Serve as a liaison between patient and medical support staff. Make a positive first impression on any person utilizing the Prevea Clinic services whether on the phone or in person.

Specialized Responsibilities:

  • Collaborate with Prevea providers to manage patients or families regarding prevention, early identification and treatment of mental or emotional problems.
  • Process referrals sent to Behavioral Care -- obtain and evaluate patient`s initial needs.
  • Triage referrals for acuity of care needs.
  • Phone contact with be initiated with all Prevea referrals to Behavioral Care to triage clinical needs.
  • Assess ability of Behavioral Care to provide services or need to refer patient out to community services.
  • Discuss and educate patients on insurance eligibility.
  • Assist Medicaid patients access mental health care and other social services thru community resources.
  • Identify needs for community resources and coordinate referrals as appropriate.
  • Follow up with referring Prevea provider on the outcome of the Behavioral Care contact.

Education/Experience/Certifications:

  • Required:
    • Knowledge of community and human social services.
    • Exceptional communication skills.
    • Exceptional problem solving skills.
    • Self-motivation.
    • Strong sense of accountability.
  • Preferred:
    • Experience work in medical clinic.
    • 2+ years` experience working in human service field.

Non-Essential Duties:

Typical Working Conditions:

Normal office environment. Occasional exposure to communicable diseases and other conditions in a clinic environment. Occasional exposure to aggressive patients.

Typical Physical Demands:

Requires sitting for long periods of time. Occasional bending, stretching or lifting. May require contact with individuals with communicable diseases. Requires the ability to work under high stress conditions for long periods of time. Requires eye-hand coordination and manual dexterity sufficient to operate a keyboard, photocopier, telephone, and other office equipment. Requires normal range of hearing and eyesight to record, prepare and communicate appropriate reports. Vision must be correctable to 20/30 and hearing must be within normal range.
