We are conducting a specialized search for an experienced Case Manager for this Long Term Acute Care Hospital located in the Portland, Oregon area. In this role the Case Manager will coordinate clinically complex patients care across a continuum; ensuring and facilitating the achievement of quality clinical and cost outcomes, negotiating, procuring and coordinating appropriate services and resources needed by the patients, and at key points, intervening to address and resolve issues/concerns. The Case Manager will also be key in ensuring appropriate sequencing of treatment goals and implementation of a comprehensive discharge plan. Demonstrate knowledge and skills necessary to provide case management services to the patient population ranging in age from 18 years to geriatric experiencing medically complex conditions, general medical-surgical conditions with rehabilitation and wound care needs.
Position requirements include: Bachelors or Masters Degree preferred. RN with CRRN, CIRS, or CCM preferred. Two through three years acute care hospital case management or insurance-based case management experience. Current CPR, and immunizations (MMR, DPT, Varicella, Hep B and TB screening) are required. Must possess strong clinical assessment and process skills. Knowledge of regulatory standards and compliance requirements.
