POSITION OVERVIEW:
Performs position duties within the Care Coordination Services department in the Post-Acute Services Division. This position is responsible for providing care coordination and utilization management activities for assigned unit and/or patients. Obtains insurance medical necessity authorizations. Facilitates appropriate level of care determinations. Participates in collaborative discharge planning. Serves as a resource to hospital and medical staff.
ESSENTIAL JOB FUNCTIONS:
· Performs Nurse Case Management activities.
· Performs utilization management functions of the Case Management Department.
· Serves as a resource to the hospital and medical staff on Utilization Management.
· Participates in implementation of MRHC Swing Bed Program.
· Promotes and supports all Team Mercy standards of quality customer service.
· Promotes continuous quality improvement in operations and in job performance.
JOB REQUIREMENTS:
EDUCATION REQUIREMENTS
Current RN license in State of Kansas. BS in nursing or related health field is recommended. Certification in Case Management is preferred.
EXPERIENCE REQUIREMENTS
Five years clinical nursing experience. Demonstrated ability to analyze patient assessment data in relation to continuum of care needs. Demonstrated effective communication skills.
KNOWLEDGE/SKILLS/ABILITIES
Extensive clinical background and knowledge of patient care processes. Excellent communication skills. Ability to self-motivate and work independently. Requires typing skills, and ability to work with computer information systems. Knowledge and experience with team leading will be helpful.
PHYSICAL REQUIREMENTS
Ability to use the telephone to communicate with departmental customers. Ability to lift 35lbs un-assisted and 75 lbs assisted. Job involves travel between departments and MRHC Campuses.Desk or computer work for extended periods oftime in office or on patient unit. Hand dexterity to use keyboard.
