Job Details:
Provide social work, discharge planning and post-acute care placement planning based on assigned patient populations/referrals from Case Manager. Identify and initiate interventions for patients in need of entitlement programs to ensure safe, effective discharge plan. Collaborate with and inform patient, family and interdisciplinary team of patient’s benefits and available community resources. Coordinate with interdisciplinary team in identifying alternate plans of care. Serve as patient advocate.
Requirements:
Bachelor's degree in Social Work from an accredited college required. Minimum of 3 years Social Work experience related to discharge planning, placement and psycho-social interventions. Working knowledge of community resources and services, entitlement programs, levels of care and Federal/State regulations specific to the elderly, disabled, and other identified populations.
