The Advanced Practice Provider works in coordination with physicians and other advanced practice providers to care for patients clinical needs and related follow-up care. This is performed in the settings of hospital call and hospital rounds.
Essential Job Functions
Demonstrates a thorough and current knowledge of disease processes while conducting a comprehensive health history and physical examination or problem-focused assessment depending on patients status.
Assess, diagnose, treat and manage patients healthcare problems/needs. Apply clinical interventions, evaluate outcomes, and implement changes to the plan of care. Anticipates and communicates changes in patient status to meet the needs of the patient.
Performs and documents ongoing patient assessment and formulates plan of care in collaboration with medical staff.
Makes hospital patient visits (rounds), as directed by the physician and/or independently, to monitor patient`s current physical condition i.e., reviews patient`s medical record, talks to patient, performs appropriate physical examination, assesses changes in patient`s condition, adjusts patient`s care plan accordingly, writes new patient orders for medications, lab/x ray tests, etc., and documents all of the above in patient`s medical record. Performs all activities in accordance with established policies and protocols i.e., all orders supervised and countersigned by physician, etc.
Conducts and coordinates patient`s hospital discharge process as directed by the physician i.e., informs and instructs patient on current condition and follow up care including outpatient, therapy and services, usage of medications, etc.; interacts with family members and/or other health care professionals relative to patient needs,. etc.; all in accordance with established policies and protocols.
Exhibits a thorough understanding of cardiovascular medications.
Monitor stress tests.
Completes thorough, timely documentation in accordance with facility policy.
Requires some experience working as either a Nurse Practitioner or Physician Assistant. Prefer at least two years of this
experience in Cardiovascular Medicine or Internal Medicine/Family Practice. Nursing or other clinical experience with adult cardiac
patients prior to graduation from a Masters program very beneficial.
Work Hours: Will work 4 - 10 hour shifts, 40 hours a week at the hospital as an inpatient APP.
