Job Summary:

The Patient Navigator works directly with adolescent, adult, and geriatric patients undergoing active treatment in an
outpatient setting. The Patient Navigator collaborates with the healthcare team in the management of biopsychosocial-financial resources to achieve optimal outcomes for the patient. The Patient Navigator will work with patients and family members through their active, palliative, and end of life care in accordance with accepted hospital standards and policy. The Patient Navigator consistently employs patient and family-centered care to promote safety, quality, and satisfaction.

Job Description:

Essential Functions:
  • Builds awareness in the patient of all available services, assists in clarifying choices and ensures unrestricted access to all services.
  • Coordinates patient information with community physicians and customizes patient schedules.
  • Initiates meetings and/or makes arrangements with support departments to meet the needs of patients; finance, counseling, socialservices/child issues, etc.
  • Initiates meetings/communications with physician offices or related professional personnel to build or maintain communication to increaseawareness and/or address processes for the patient navigator program
  • Organizes and creates a plan customized for each patient referred; scheduling, supportive resources (i.e. counseling, dietician, wigservices), and post-treatment resources (i.e. support groups).
  • Participates with multidisciplinary team meetings discussing future and present patient cases and collaborates efforts to meet needsidentified.
  • The Navigator coordinates data, community physicians, and Regional Health resources into a seamless model of access and carebenefiting patients, participating physicians, and family members.
  • Works with staff, administration, and physicians to match resources with patient needs.
  • All other duties as assigned.

    Additional Requirements:

    Required:
    Certification - Registered Nurse (RN) - South Dakota Board of Nursing

    Preferred:Education - Bachelors degree in NursingExperience - 1+ years of Case Management Experience; 1+ years of Patient Care ExperiencePhysical Requirements:
    Light work - Exerting up to 20 pounds of force occasionally, and/or up to 10 pounds of force frequently, and/or a negligible amount of force constantly to move objects.Make a difference.
