This position will work 30 hrs per week

Job Description and Essential Functions

Job Title/Position: Registered Nurse 2 -- Addiction Nurse

Job Classification: Direct Patient Care / Hands-On Patient Care

Brief Description of Job Responsibilities:

  1. Telephone triage
  2. Give patients telephone advice using nursing judgment within the scope of license
  3. Communicate with providers about patient calls received
  4. Triage patients to the appropriate setting if there is a need to be seen
  5. Provide patient education
  6. Educate patients and families about managing their condition and/or disease
  7. Report test results to patients and instruct as to the appropriate follow-up
  8. Call in prescription refills as directed
  9. Assist in scheduling patients
  10. Communicate with hospitals and other offices as directed by providers
  11. Room patients
  12. Assist with procedures as needed
  13. Administer injections
  14. Provide direct observation of urine drug screen collections as needed
  15. Prepare charts for assigned patients prior to appointment
  16. Conduct pre-visit planning with providers
  17. Write individual care plan with treatment goals
  18. Assess patient barriers to successful outcomes as it relates to medications, appointments, and goals
  19. Coordinate visits with multiple clinicians and/or diagnostic tests
  20. Identify health insurance or financial resources for patients
  21. Review clinical measurements with patients at visits
  22. Support provider in addiction clinic
  23. Monitor patients currently in addiction program
  24. Assess new patients for eligibility to participate in the addiction program
  25. Provide direct observation of urine drug screen collections
  26. Write individual care plan with treatment goals
  27. Review information from facilities (discharge summary, hospital admission or ER visits) to determine of patients require a follow-up visit or who are at risk for adverse outcomes
  28. Engage n AODA patient privacy requirements
  29. Collaborate with physician, AODA counselor, internal and external case managers and referring hospitals, county agencies and clinics
  30. Obtain appropriate prior authorizations needed for medications prescribed through addition clinic.
  31. Perform related work as required/requested by the supervisor/coordinator

Specialized Responsibilities:

Responsible for coordinating the continuum of care for the Hope Grant program patients and ensure optimum utilization of resources, excellent service delivery and positive quality outcomes, while minimizing unnecessary medical expenses

Performance Requirements:

  1. Knowledge of professional nursing theory and practice to give and evaluate patient care.
  2. Knowledge of organizational policies, relations and procedures to administer patient care
  3. Knowledge of medical equipment and instruments and administer patient care
  4. Knowledge of common safety hazards and precautions to establish a safe work environment
  5. Skill in applying and modifying the principles, methods and techniques of professional nursing to provide on-going patient care
  6. Skill in identifying problems and recommending solutions
  7. Skill in preparing and maintaining records, writing reports, and responding to correspondence
  8. Skill in developing and maintaining department quality assurance
  9. Skill in establishing and maintaining effective working relationships with patients, medical staff, and the public
  10. Ability to maintain quality control standards
  11. Ability to react calmly and effectively in emergency situations
  12. Ability to interpret, adapts, and applies guidelines and procedures
  13. Ability to communicate clearly

Education/Experience/Certifications:

  1. RN with current Wisconsin license
  2. Current CPR Certification
  3. Previous experience with addiction services and patients who have alcohol or drug dependency, preferred
  4. Knowledge of opiate and alcohol symptoms and assessment scales including Clinic Indicator Withdrawal Scale (CIWA) and Clinical Opiate Withdrawal Scale (COWA), preferred

Non-Essential Duties: Other duties as assigned within scope of practice

Typical Working Conditions:

The typical working conditions are in clinical office and exam room environments. Work involves phone contact with a various array of patients, employees, and providers. Work may be stressful due to multiple calls and emergency situations. Possible exposure to communicable diseases and medicinal preparations common to a clinic environment.

Typical Physical Demands:

  1. Requires a full body range of motion with handling and lifting patients
  2. Requires standing and walking for extended periods of time
  3. Occasionally lifts and carries items up to 50 lbs. (linens, supplies)
  4. Push and pull patients in wheelchairs occasionally with up to 75 lbs. of force
  5. Requires visual acuity correctable to 20/30 and hearing within normal range and color perception

This position will require working on outreach adn telemedicine project which include travel
