We are searching for a Registered Nurse whose primary responsibility is to provide direct nursing care for our patients. The services provided are determined by working in close consultation, and collaboration with, the Primary Nurse-Case Manager. The Residence RN must be able to communicate effectively with patients, caregivers, and other health professionals. This position reports to the Residence Manager, or designee.

We are searching for a Registered Nurse whose primary responsibility is to provide direct nursing care for our patients. The services provided are determined by working in close consultation, and collaboration with, the Primary Nurse-Case Manager. The Residence RN must be able to communicate effectively with patients, caregivers, and other health professionals. This position reports to the Residence Manager, or designee.


RESPONSIBILITIES:

  • Reports any significant changes in the patient`s status to Primary Nurse-Case Manager, or designee.
  • Implements Care Plan as established by Primary Nurse-Case Manager and
  • completes nursing assessments.
  • Accurately administers prescribed medications and performs treatments as ordered.
  • Supervises Residence Aides or licensed practical nurses (LPNs).
  • Assists agency Medical Director and/or attending physicians as needed.
  • Assures completion of, or directly provides, patient hygiene, feeding, and laundry.
  • Works closely with other members of the Interdisciplinary Team (IDT) to enhance careand support for patients and caregivers.
  • Monitors the supply of medications and participates in ordering medications as
  • appropriate.
  • Documents, counts, and disposes of medications per agency policy.
  • Prepares and presents change of shift report.
  • Assures that coverage for next shift is present before leaving the Residence.
  • Receives Residence staff call-ins and initiates agency procedure to ensure shift coverage.
  • Works closely with Residence Volunteers.
  • Maintains up-to-date knowledge of the Residence specific agency policies and
  • procedures, including health, fire, and safety measures.
  • Participates in meal preparation, and preparing grocery list as needed.
  • Monitors supply inventory and reports needs to Clinical Administrative Assistant, or designee.
  • Documents within the Electronic Medical Record (EMR) all care provided in an
  • appropriate and timely manner.
  • Attends staff meetings as required.

QUALIFICATIONS

REQUIREMENTS:

  • Maintains a current license in NYS to practice as a Registered Professional Nurse.
  • Must obtain Hospice and Palliative Care Nursing certification within two years of hire.
