Job Description:

The Staff RN for inpatient is responsible for the provision of comprehensive Nursing care to meet the needs of his or her patients. Under the direction of the Inpatient Manager, the registered nurse has the authority and accountability for utilizing the nursing process and delegating components of the nursing plan of care to allied nursing personnel. This includes participation in the Interdisciplinary Group to provide a comprehensive plan of patient care.


Responsibilities:

  • Systematically and continuously collects and documents assessment data regarding patient and family needs, identifying areas that require health care intervention.
  • Communicates the patient or family needs to the interdisciplinary group and assists in developing a comprehensive care plan for the patient and family.
  • Develops, implements, and evaluates goal directed plans of nursing intervention based on identified needs in collaboration with the attending physician and other health team members.
  • Manages and coordinates complex as well as routine patient care in manner which ensures the efficient delivery of that care.
  • Communicates with the Community Hospice Nurse and the Hospice Social Worker to assess the need for home health services and to develop the home care plan.
  • Communicates all changes in patient status and or service needs to the appropriate health team members and ensures appropriate action is taken.
  • Facilitates the development and implementation of transfer to the home setting.
  • Documents all patient care and coordinating activities in a concise, accurate, and timely manner.
  • Provides support and direction to the family at the time of the patient`s death. May be involved in the family`s bereavement follow-up.
  • Maintains an updated working knowledge of developments in nursing practice and community support services.
  • Participates in utilization review and/or quality assurance activities as requested.
  • Attends required staff meetings, in-services, and/or supervisory conferences when required.

Rochester is a city on the southern shore of Lake Ontario in western New York with a population of 208,046 residents. Rochester is the seat of Monroe County and the third most populous city in New York state, after New York City and Buffalo.
It is home to numerous cultural institutions and hosts a number of festivals every year. Popular is the Lilac Festival held for ten days every May, when more than 400 varieties of lilacs bloom, and hosts 100,000 visitors.
Lake Ontario, the Erie Canal and numerous parks provide numerous summer and winter sports such as boating, fishing kayaking, camping, tubing, skiing, skating, etc.

Niagara Falls, a national treasure, is located only 1.5 away. Southeast is the Finger Lakes and wine country.

QUALIFICATIONS

Requirements:

Graduate of an accredited school of nursing and valid NYS registered nursing license required.
A minimum of 1 year recent home health care agency or acute care experience preferred.
Hospice or palliative care nursing experience helpful.
IV/Infusion skills may be required.
Specialized clinical experience and skills may be required for certain team positions.
Must demonstrate solid interpersonal, organizational and time management skills.
Basic computer skills are necessary, including but not limited to the ability to navigate on a personal computer within a Windows based operating environment; the ability to access and create basic e-mail messages; and the ability to open, edit, and save basic electronic documents if necessary.
With training provided, must demonstrate the ability to learn and utilize new systems, computer applications and operating environments as necessary.
Must have ability to travel.
An interest in the care of the terminally ill and a desire to care for the family of the patient.


Requirements:

Skills Preferred

  • Health Care Intermediate
  • Home Care Novice

Education Preferred

  • Associates or better in Nursing.
  • Bachelors or better in Nursing.
  • Nurse, RN or better in Nursing.

Licenses & Certifications Preferred

  • Registered Nurse

Education:
University - Associate`s Degree/Graduate Diploma/2 Years
