Summary

Provides physician ordered physical therapy services, which include assessment, planning, developing, and implementing an individualized patient therapy plan. This position coordinates the physical therapy plan with the interdisciplinary team care plan.

Responsibilities:

  • Provides physician ordered physical therapy services.
  • Assures orders are renewed, per schedule, or as patient`s response to current orders change.
  • Is responsible for completing, in a timely manner, all documentation including initial assessment, visit notes, renewal orders, verbal orders, and discharge summaries.
  • Identifies patient short/long term goals in terms of functional outcomes. Reflects progress or lack of goal achievement in all charting. Confers with physician and/or interdisciplinary team regarding patient response to therapy.
  • Evaluates home environment for changes needed to increase patient`s independence. Recommends equipment and assistive devices (ramps, bathroom aides, adaptive wheelchair), as appropriate.
  • Instructs the patient and caregivers in therapeutic exercise, which may be used to improve the patient`s functional status.
  • Provides gait evaluation and ambulating training. Select and instruct in the use of various assistive devices (orthotic appliance, crutches, walker, cane, etc.).
  • Treats patient`s physical deficits using clinically accepted therapeutic exercises. Specifies technique used in the plan of care.
  • Evaluates and instructs patient and caregivers in safe transfers (bed, chair, commode, etc.) using appropriate body mechanics and equipment (sliding board, hoyer lift, trapeze, bath bench, etc.).
  • Attends scheduled interdisciplinary staff meetings and actively participate in treatment planning.

Rochester is a city on the southern shore of Lake Ontario in western New York with a population of 208,046 residents. Rochester is the seat of Monroe County and the third most populous city in New York state, after New York City and Buffalo.
It is home to numerous cultural institutions and hosts a number of festivals every year. Popular is the Lilac Festival held for ten days every May, when more than 400 varieties of lilacs bloom, and hosts 100,000 visitors.
Lake Ontario, the Erie Canal and numerous parks provide numerous summer and winter sports such as boating, fishing kayaking, camping, tubing, skiing, skating, etc.

Niagara Falls, a national treasure, is located only 1.5 away. Southeast is the Finger Lakes and wine country.
3.5 hours to Adirondack Mountains

QUALIFICATIONS

Qualifications:

  • BS or MS degree from an accredited Physical Therapy school with current NYS licensure.
  • Minimum of 1 year experience in an acute care facility and/or an acute intermediate rehabilitation facility.
  • New therapists or new to homecare will be placed in the lower level of this job.
  • Level two is meant for those with considerable homecare experience and demonstrated higher level of competence.
  • Ability to travel required.
  • Possession of a valid NYS motor vehicle operator`s license.
  • Pre-employment physical examination and an annual health assessment as require by New York State Health Department.

Level II - requires similar qualifications as level I, plus:

  • Demonstrated leadership skills.

Education Required

  • Bachelors or better in Physical Therapy.

Preferred

  • Masters or better in Physical Therapy.

Licenses & Certifications Required

  • Physical Therapist

Education:
University - Bachelor`s Degree/3-4 Year Degree
