Job Summary: Works in collaboration with the Medical Director and Hospice Team to provide Hospice care services to patients and families in the inpatient setting or in a home setting within the scope of practice of a nurse practitioner. Perform procedures for which he/she is privileged to do and documents assessments, evaluations, consultation reports and progress notes. Demonstrates Competency in the Following Areas: Performs face-to-face encounter with the patient as a part of the certification process; completes Attestation documentation. Collects and assesses patient care data, develops and implements a plan of care, manages and evaluates outcome of care. Performs physical and psychosocial assessment on all Hospice patients. Ability to adequately assess and reassess pain. Utilizes appropriate pain management techniques. Educates the patient and family regarding pain management. Assists in developing a pain management and symptom management plan to keep the patient as comfortable as possible. Reassess pain management plan when pain is not manageable on the current medications. Demonstrates the ability to prescribe medications (see individual state laws) and non- pharmacological therapies based on the patient's diagnoses, health history and age. Regulatory Requirements: Current Registered Nurse License in the State of Georgia. Master's Degree from an accredited Nurse Practitioner's Program. National Certification as a Nurse Practitioner or certification eligible. Current BCLS Certification. A minimum of one (1) year Nurse Practitioner experience preferred. Previous Hospice experience preferred.
