Summary & Objective

The Palliative Care Medical Assistant works with patients, clinicians, and multidisciplinary team members to assess, facilitate, plan and coordinate delivery of care. Ensures timely transitions to minimize redundancy in handoffs and provides consistency for patients.


Essential Functions

  • Ensures effective communication assistance with answering phones, scheduling, and other requests.
  • Acts as patient liaison in helping patients feel at ease and able to explain nuances of the palliative services.
  • Schedules patient and family visits for the purposes of goal setting, advance care planning, support, and plan of care development, based on the patient's needs, values and preferences.
  • Documents patient information by interviewing patient, recording history, medications, allergies, screening tools and confirms purpose of visit.
  • Obtains record releases and coordinates obtaining patient records prior to visits.
  • Secures patient information and maintains patient confidence by completing and safeguarding medical records.
  • Prepares chart for clinicians prior to visits with all documentation available.
  • Assists clinicians as requested with examination needs, supply preparations, and support.
  • Explains treatment, procedures, medications, diets, or any other clinician instructions to patients, as directed and supervised by the clinical team.
  • Communicates with referral sources by providing documentation of visits timely.
  • Documents all conversations with referral sources, patients, and families appropriately in the patient's record.
  • Assists with correspondence, billing, insurance verification and other administrative tasks.
  • Accepts referrals from all sources, not limited to phone orders, including physicians, social workers, insurances, case managers, patients, and family members.
  • Participates in the development of processes to improve the clinical experience for referral sources, patients/families, care management teams, etc. across the continuum of care.

Other Duties

  • Ability to coordinate several projects at the same time with competing demands and tight timelines.
  • Ability to handle sensitive information and maintain confidentiality.
  • Demonstrates knowledge of policies and procedures and pertinent State and Federal Rules and Regulations.
  • Ability to educate staff and respond to diverse referral sources regarding questions pertaining to palliative care or hospice services.
  • Able to maintain files and other information in an organized manner.
  • Ability to interact with the public and other employees in a positive manner and promote a cooperative working environment.
  • Maintains required licenses, certifications, and mandatory skill updates.
  • Adheres to standard precautions and transmission-based infection control procedures, utilizing appropriate Personal Protective Equipment.
  • Complies with HIPPA requirements.
  • Participates and complies with all deadlines as required for Annual Education.
  • Participates and complies as required for Employee Health Screenings.
  • Participates and complies with Meeting Attendance as required.
  • Comply with all policies, local, state, and federal laws, and regulations.
  • Perform other duties as assigned.

Physical Requirements

  • Must be able to lift and/or move up to 50 pounds and push/pull up to 250+ pounds, walk, climb stair or ladders, stand on feet for extended periods of time, etc.

Knowledge & Experience Requirements

  • LPN, Certified Clinical Medical Assistant (CCMA) or equivalent education or equivalent combination of education and experience.
  • 2 years of clinical and administrative experience in physician practice required.
  • Prior palliative or hospice care experience preferred.
  • Must have knowledge of computer office/clinical software and practice management systems.
  • Must be able to read, write and understand the English and Spanish language.
