Registered Nurse (RN) Description:

  • Under the direction of the DON, contributes to planned nursing care to achieve identified outcomes
  • Provides quality home health care to clients
  • Provides a skilled nursing assessment of all systems, noting any changes in condition and reporting them to the DON
  • Maintains accurate and legally appropriate documentation of nursing care, actions, and observations, and ensures the responsible documentation of nursing care and health orders

Registered Nurse (RN) Procedure:

  1. Reviews/audits client information sheet for changes
  2. Reviews previous 485 and all MD orders
  3. Reviews all nursing notes for client progress, med changes, and information to be included in MD summary
  4. Reviews/updates med profile. Includes any new diagnoses, ensures primary Dx is up to date, and ensures that all Dx are listed
  • Prepares and maintains clients Plan of Care per certification period and updates as needed. Reports any and all changes to the Director of Nursing
  • Prepares and administers medications as ordered by the physician
  • Updates medication profiles as changes occur and updates the profile for every certification period for the chart and displays it in the clients home
  • Assesses the need for an increase or decrease in skilled nursing visits due to change in status and reports any and all changes to the Director of Nursing
  • Updates the Client Care Plan as care needs change and provides a copy of revised Client Care Plan to the Director of Nursing and the Scheduling Manager
  • Assesses the clients functional/safety status and notifies the Director of Nursing of any changes in equipment or therapy needs
  • Communicates with MDs to report client conditions, obtain orders, and report lab results
  • Consults with the DON, client, family, Authorized Representative, and/or the attendant as medical issues arise

Registered Nurse (RN) Requirements

  • Graduate of an approved RN program with a current, unencumbered license to practice as an RN in the state of Colorado
  • Current CPR certification is required for RNs performing Private Duty Nursing
  • Home health experience is preferred
  • Reports to work consistently and on time
  • Has a valid driver's license, current auto insurance, and a reliable vehicle, or a workable plan for alternate transportation

Registered Nurse (RN) Knowledge

  • Must have good knowledge of nursing and medical techniques and procedures
  • Knowledge of Medicaid/Medicare rules and regulations, as well as regulations and guidelines that pertain to home health care. Maintains an effective working knowledge of AOIs policies and procedures
  • Knowledge of principles and processes for providing exceptional client services, including client needs assessment, meeting quality standards for services, and evaluation of client satisfaction

Registered Nurse (RN) Skills

  • Must have the skills to observe, identify, report, evaluate, and document clients progress and reactions to care
  • Gives full attention to what other people are saying, taking time to understand the points being made, asking questions as appropriate, and not interrupting at inappropriate times
  • Must possess excellent organizational skills and have a strong attention to detail

Registered Nurse (RN) Ability

  • Must be able to develop procedures for work activities and carry work through to completion using own initiative with minimal supervision
  • Able to develop constructive and cooperative working relationships with others, and maintain them over time
  • Accepts accountability and responsibility for own actions within the nursing practice. Possesses the ability to deal tactfully with clients, the clients medical doctor, family and friends of the client, and the general public. Must have the ability to work with difficult clients
