Responsible to:
Director of Business Services and Coding Departments
Job Summary:
Under the direct supervision of the Coding Director, the Medical Coder is responsible for assigning the appropriate ICD-9, and CPT codes to patient charts and entering charges according clinic policy. CPC or RHIA certification required.
RESPONSIBILITIES:
1. Obtain hospital day sheets from assigned physicians daily, marked with appropriate levels of service, and diagnosis codes.
2. Access hospital system to verify dates of admit/discharge and obtain complete patient demographics.
3. Retrieves and reviews patients medical records from hospital, C&D medical records, Auto Chart (Medic), or from physician, when necessary to process charges to completion.
4. Utilizes ICD-9 (diagnosis), CPT (procedural), HCPCs, specialty-focused books, and CCI (Correct Coding Initiative (bundled procedures) in order to ensure that charges are posted accurately to avoid unnecessary insurance denials.
5. Checks procedure to diagnosis requirements to ensure procedure is payable with the assigned diagnosis code in order to ensure prompt reimbursement.
6. Determines and records other required medical information.
7. Responsible for knowledgeable of current coding guidelines, as well as insurance guidelines and regulations regarding proper coding and reimbursement.
8. Works with clinical and business office staff in coordinating medical information and patient charts.
9. Checks Medic system to make sure that the patients primary physician (or another physician has not billed similar services on the same dates with the same diagnosis codes.
10. Posts all hospital charges from assigned physicians by close of month (2nd business day of month to avoid denials for timely filing, which would result in unnecessary write-offs against doctors production.
11. Recognizes charges, which must be filed to insurance carriers as paper claims with documentation attached (i.e.: 22-modifier).
12. Prints paper claims and attaches documentation necessary , and files with insurance carriers.
OTHER DUTIES:
1. Assists with special projects as assigned by manager.
2. Adheres to all C&D policies and procedures.
3. Performs other related duties as requested by manager.
QUALIFICATIONS:
1. High school diploma.
2. CPC or RHIA certifiation
3. Typing skills.
3. Computer skills.
4. Knowledge of medical terminology, CPT, ICD-9, and HCPCs coding procedures.
5. Anatomy and physiology.

1. Must be willing to effectively participate as a team member with the clinic staff.
2. Must be committed to the protection of confidential information, records and/or reports.
3. Must have the ability to communicate with physicians and staff members effectively in a professional and courteous manner
4. Must have excellent organizational skills.
ENVIRONMENTAL FACTORS:
1. Requires sitting for long periods of time.
2. Requires some lifting and walking.
3. Requires manual dexterity sufficient to operate a keyboard.
4. Vision must be correctable to 20/20 and hearing must be in normal range for telephone contacts.
TYPICAL WORKING CONDITIONS:
1. Normal office environment.
PERFORMANCE REQUIREMENTS:
Knowledge, Skills, and Abilities
1. Must be proficient within the Medic system and other applicable computer applications (i.e.: LRMC hospital system).
2. Must be able to retrieve patient medical records (charts) from C&D record department and locate necessary documentation in order to process charges, or file claims to insurance carriers.
3. Must have knowledge of medical terminology, anatomy and physiology.
4. Must have working knowledge of ICD-9, CPT coding guidelines.
5. Responsible for accurate coding to ensure maximum reimbursement.
6. Completes work load by end of month (close).
