Job Summary:
Under the supervision of the Manager of Health Information, the Clinical Documentation Specialist facilitates improvement in the overall quality, completeness, and accuracy of medical record documentation. The specialist will clarify clinical documentation through extensive interaction with physicians, nursing staff, other allied health providers and the Health Information Management (HIM) coding staff to ensure that the clinical documentation reflects the severity of illness and level of service rendered to patients. This position is responsible for the ongoing education of physicians and other members of the patient care and coding teams on compliant documentation and coding guidelines. Responsible for activities related to recovery audits (RAC) and DRG validation requests. Provides input into performance reviews for hospital coding staff.
Job Requirements:
Minimum of 4 years of experience in Health Information Management with a primary emphasis on coding and documentation. Minimum of 2 years of supervisory or team leader experience preferred. Excellent knowledge of ICD-9, CPT, APC and DRG assignment, anatomy and physiology, and medical terminology. Excellent understanding and application of AHA coding guidelines. Effective observation, analytical, and critical thinking skills. Effective problem solver with excellent interpersonal and communication skills, both verbal and written.
